Bill of Lading

We have a simple bill of lading form that you can fill out online. Please click here for our Bill of Lading form. The form can be printed with the information you input on screen, or just print it out blank and fill it in by hand. Freight to and from Hawaii usually gets from origin to destination with more than one mode of transportation, so this bill of lading form is slightly different than most. Note that all shipments with MFS Freight Services are subject to our Terms of the Bill of Lading, as shown on page two of the form.

Credit Application

Click for a form that can be filled out on the screen. Although not visible on the screen, there are input fields on each line. Just click on a line and you can use the tab key to move between fields. When done, just print it, sign it and send it to us. You may also print the form and fill it out by hand.

Claim Form

Click to submit a claim for loss or damage to a shipment. This form can be filled in on line and printed or saved to your computer for future use.

Vendor ACH Payment Form (Credit Card)

Vendors can use this form to elect payment via direct deposit. No waiting for a check or wasting paper.

W-9 Form

This form contains MFS Freight Services tax payor ID number.
